Walk into any tennis club on a Saturday morning and count the elbow straps. You will not have to look hard. Now think about the last professional match you watched — how many of those players were wearing one?
That gap is the whole story. Tennis elbow is overwhelmingly a recreational player's problem, and it is not because club players train less or care less. It is because of how the arm is being used, how the ball is being struck, and — the part most players never examine — what they are holding while they do it.

Why elite players almost never deal with it
Two things separate a touring professional's arm from a club player's arm, and neither of them is fitness.
The first is that the arm never works in isolation. When a high-level player strikes a ball, there is an effortlessness to it that is easy to misread as relaxation. It is not relaxation. It is the legs, hips, trunk, shoulder and arm arriving in sequence, so that no single link in the chain is carrying the load on its own. The recreational version of the same shot often skips the first three links. The arm supplies the power, the arm absorbs the return, and it does this a few thousand times a week.
The second is contact quality. Professionals mishit too — they frame balls and they catch the ball off-centre. They simply do it far less often. And the difference matters more than most players assume: research on one-handed backhands has shown that where the ball strikes the string bed, together with how tightly the handle is being held, changes the load that reaches the arm. Off-centre contact twists the frame in the hand. That twist has to be resisted, and the muscles that resist it attach at the outside of the elbow.
A club player hitting 30 off-centre balls a session is asking the forearm to do a job the pro's arm is simply not being asked to do.
You will often hear that half of all recreational players suffer from tennis elbow. The published epidemiology is more modest: a review in the International Journal of Racket Sports Science reports elbow injury prevalence in tennis at roughly 1.3–14.1%, with no meaningful difference between men and women, and puts incidence in the general population at around 2%, most commonly diagnosed between ages 40 and 53. The problem is real and concentrated in exactly the adult recreational demographic — it is just not universal.
Technique, and why it is usually the backhand
Tennis elbow refers to pain on the outside of the elbow, and the strokes that load that side hardest are the one-handed backhand, the backhand slice and the backhand volley. On those shots the impact is absorbed on the outer forearm. On the serve and forehand the load runs more through the inside of the arm, which is why golfer's elbow — the inner-elbow version — is more commonly associated with those strokes.
Two caveats keep this honest. A two-handed backhand shares the load across both arms and is much less implicated. And once you already have tennis elbow, everything hurts. The pain does not politely restrict itself to backhands; serves and forehands will aggravate it too. That is a symptom of the condition, not evidence about its cause.
Your grip size — and why too big is the dangerous direction
This is the cheapest thing on the list to check and the one most often wrong. Grip size that does not match your hand changes how hard you have to squeeze to keep the racket stable, and squeeze pressure is the direct input to the forearm extensors.

Your fingertips are pressing into your palm. This is the less harmful error, and plenty of players compete happily with a slightly small grip. Watch the thumb: it should rest on the middle finger, and if it starts to overlap, that introduces instability.
This is the direction that causes trouble. You cannot wrap your hand fully around the handle, so you squeeze harder for the same stability — every ball, every set, all season. At club level, oversized grips are the more common error.
Why this matters more than it sounds. Handle size is one of the few racket variables with a direct published link to the muscle group involved in tennis elbow. Laboratory work on grip size and forearm loading has found the relationship is U-shaped: both undersized and oversized handles raise the grip force required, and the load on the wrist extensors along with it. There is a middle condition, and it is worth finding yours before changing anything more expensive.
Grip size is only half of it — the other half is friction
Two players can hold identically sized handles and squeeze completely differently, because size is not the only thing that determines how secure a racket feels. Grip is the product of size and friction. A correctly sized handle that slips in a sweaty hand in the third set will still get squeezed, and squeeze pressure is the input that reaches the forearm extensors.
This is the layer most players never touch. You replace your overgrip regularly; almost nobody ever looks at what is underneath it. That base layer is what the racket is actually anchored to, and on most frames it is the same generic factory grip that shipped in the box.

Every one of those frames is stiff. Several are among the stiffest ever mass-produced. By the softness-equals-comfort rule they should be arm-wreckers, and yet they are the rackets that keep 70-year-olds on court four times a week. The reason is that light plus head-heavy plus stiff plus oversized adds up to enormous free power. The ball leaves the string bed with very little contribution required from the player. Less effort per shot, thousands of times.
Softness is one variable out of five. The other four are all asking the player to generate more of the ball themselves.
The marketing contradiction, in one brand's own catalogue
Take a single manufacturer's line. One frame is described as an arm-friendly player's racket, and the arm-friendliness is attributed to its 64 RA stiffness. Elsewhere in the same family sits a 115 sq in version marketed for maximum comfort and power — with a 70 RA rating. Both are sold on comfort. They point in opposite directions on stiffness.

The softer frame is 71 grams heavier, shorter, smaller-headed and more head-light. If you handed both rackets to someone with a sore elbow, the stiffer one would very likely produce the easier afternoon.
The wooden racket problem
The cleanest argument against softness-as-cure is historical. Wooden rackets had stiffness ratings somewhere around 30 — unimaginably flexible by modern standards. Players still got tennis elbow. Whatever is going on, extreme flexibility on its own was never the answer.
There is a physical account of why. A flexible frame bends and warps more on impact and vibrates for longer, and energy that goes into bending the frame is energy that does not go into the ball — so you swing harder to compensate. That reading points toward a stiff frame at low string tension as the more comfortable combination, which is close to the opposite of the usual shop-floor advice.
What to change if your elbow already hurts
First, the rule that overrides everything: if you like your racket and you are playing pain-free, do not change it. Professionals switch frames rarely — many play a paint job of an older mould rather than the model on the shelf bearing their name. There is no prize for churning equipment.
If you do have pain, the four adjustments below work as a package. Applied individually they can easily cancel each other out.
Under 100 sq in, move to 100. Already at 100, look above it. Bigger head, bigger sweet spot, more free power.
Head-light frames are more manoeuvrable but generate less on their own — which means you generate more.

Strings and tension
Strings are the cheapest meaningful change you can make, and tension is the cheapest of all — it costs nothing but a note to your stringer.
Tension first, because it is the bigger lever. Club rackets are routinely strung far tighter than they need to be. A tighter bed is a stiffer, deader bed, which means more effort per shot and more shock returning up the handle. With polyester, stringing in the 40s is normal and the high 30s is workable depending on the string and the frame. Dropping tension is reversible, free to try, and frequently the single change that people notice most.
On string type, the logic follows the racket argument. The property to look for is responsiveness — a string that gives the ball back rather than swallowing it. Not every polyester qualifies; some are genuinely low-powered and unpleasant. But a good monofilament copolyester in a thin gauge, strung loose, can be both firm and elastic at the same time. That combination is what stringers listen for: a live, high-pitched sound off the string bed rather than a dull thud.
Worth knowing: string formulations change quietly between production runs while the name on the packet stays the same. If a string worked for you five years ago and the current version feels different, you are probably right.
One thing string work cannot do is change what happens after impact. Whatever vibration survives the string bed travels down the frame and into your hand, and the only thing standing between it and your palm is the grip. It might not be tennis at all
Sometimes no equipment or technique explanation fits.
